New Patient Registration
All new patients must see the Health Care Assistant (HCA) to register with the practice. You will need to make an appointment for a 'new patient interview', which involves some simple health checks and allows us to learn about any previous medical problems, any allergies that you have and any medication you are taking. Please ring the appointment line on 01384 322500 to book this. (If you have completed an on-line pre-registration form, please allow a couple of working days before ringing for an appointment to enable it to be processed). When you attend this interview, please bring with you any regular prescription medicines (or your last repeat request ordering slip) supplied by your last Practice.
Please arrive at least 10 minutes before your new patient interview as you will not be fully registered until you have attended this interview. If you have any problems that mean you may have difficulty communicating with the Practice, or the Practice communicating with you please mention this at the interview. (e.g. you need letters in large print or braille, difficulty using the telephone etc.).
You will be allocated a 'named' GP who has overall responsibility for your care, however you are free to choose to see the doctor you prefer but in general it is better to stick to one doctor where possible, especially when dealing with a continuing problem.
The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.
We will also require proof of your identity AND proof of your address within our catchment area before we can register you. For example we will need to see and take a copy of your:
- Photo identity, such as passport or drving licence.
- Proof of address, such as a recent council tax bill or current utility bill (e.g. gas, electricilty, water or phone but not a mobile phone bill)
Medical treatment is available from the date of registration. Please contact reception for further information.
