In order to register at the practice you will need to first of all check that you are within our catchment area (please see tool below)
You will then need to complete the following forms which can be downloaded and printed or collected from the practice.

You will be asked to show proof of your identity and home address, suggested forms of ID are:
Photo identity, such as passport or driving license.
Proof of address, such as a recent council tax bill or current utility bill (e.g. gas, electricity, water or phone but not a mobile phone bill)
As a new patient at the practice you will be invited to attend an optional new patient health check with a Health Care Assistant (HCA).
Registration will only be complete 3 working days later.
Please note that it may take some time to receive notes from your previous practice, therefore it is recommended that you obtain at least one month’s supply of any regular medications you are taking from your previous practice before registering here.
Upon registration, you will be allocated a 'named' GP who has overall responsibility for your care, however you are free to choose to see any doctor of your choice. For urgent appointments you may only be able to see the duty doctor on the day.
