Pre Travel Vaccinations

  • If you require any vaccinations relating to foreign travel, initially you need to make a telephone appointment with the practice nurse to discuss your travel arrangements. 
  • This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. 
  • It is important to make this initial telephone appointment as early as possible  - at least SIX weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. 
  • The link below will take you to the Travel Risk Assessment form, it may help you have the information to hand and WILL BE REQUIRED to be completed and brought with you to the second appointment.

  • Some vaccines may have to be ordered if they are not a stock vaccine. 
  • Your second appointment needs to be at least FOUR weeks before you travel to allow the vaccines to work.
  • Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.

NHS Health Checks

If you are aged between 40 – 74 you are entitled to a free NHS Health Check every 5 years. The NHS Health Check is designed to reduce the risk of patients developing any long term conditions and provides advice on how to maintain a healthy lifestyle.

NHS Health Checks – 75 years +

If you are aged 75 or over you are entitled to a free NHS Health Check if you haven’t had one in the last year. Similarly to the 40-74 year old check, it is designed to reduce the risks of developing long term conditions and to provide advice on how to maintain a healthy lifestyle. Our staff are also trained to carry out memory assessments and frailty risk assessments.

Contraception and Sexual Health 

Comprehensive, confidential advice is available during normal surgery hours.  We offer oral contraceptives, injectable contraception, sub-dermal implants and coils.  We also offer emergency contraceptive advice. Antenatal Clinic  

This clinic is run by the midwife on Tuesdays & Thursdays.  If you become pregnant, you will be given a 'booking appointment' at which the pregnant

midwife will ask you a few questions and carry out some general health checks. 

You will be seen regularly throughout your pregnancy either at the practice or at the local hospital, or both.

Cervical Smears  

These are normally carried out by the practice nurses.  Please telephone the surgery appointment line to make a convenient appointment.  If you have any problems or queries about cervical smears please contact the practice nurse.

All new babies and Mothers are invited for a check-up when baby is six to eight weeks old.                         

Long Term Condition Management

Our Nursing team have a wide range of skills to help monitor patient long term conditions and will offer health advice in order to reduce further health risks and to enable patients to live the healthiest possible lifestyle. Long term condition reviews are scheduled to take place in the month of patient’s birthdays. This system was introduced to make it simpler for patients to remember when they are due for a check-up. Patients who require blood monitoring as part of their annual check-up should attend the surgery to collect a blood sample request form and have their bloods taken at least one week before their review is booked.

Immunisations For Babies To Pre-School                                                                                               

Baby & Childhood immunisations are carried out by the Practice Nurse, you will be invited by letter from Child Health.

If you cannot bring your child yourself you can give written permission for a relative to do so on your behalf.

Minor Surgery 

Procedures such as joint and tennis elbow injections, treatment of warts and other skin problems can often be done in our treatment room, avoiding the need for hospital referral and the delays involved.  Please ask us regarding any moles, lumps or bumps that you are concerned about, as many may be suitable for removal at the surgery.  Please discuss this with your doctor who will then arrange for you to be given an appointment.

Quit Smoking

Medical Reports and Non-NHS examinations 


Some services requested at the practice lie outside the remit of the practice's NHS contract and therefore there is a charge for these services.  Non-NHS services include: accident and sickness insurance medical reports, certificates (such as fit notes for first week of work absence); drivers', school and sports medicals, holiday insurance certificates, passport applications, private health insurance claims, private prescriptions and some vaccinations for travel abroad. 

You will be required to complete a private work application form and you may need to make a non-refundable deposit. Please speak to our reception team or secretaries for more information.

Medical Fitness for Work Certificates  

When off work due to sickness, an employee can self-certify for up to seven calendar days.

If you are ill for more than seven calendar days, certificates are obtained as part of the consultation with your doctor.  Any request from an employee/employer for a doctor's certificate covering fewer than eight calendar days will be a private certificate only, for which there will be a charge.  If you attend a hospital department and are told not to work, you must get a certificate from the Doctor there.

For on-going sick note requests you can visit the practice and complete a Sick Note Extension Request Form. The doctor may be able to continue your sick note without being reviewed each time depending on your circumstances.

Chiropody/Podiatry 

If you have a problem with your feet, the doctor may refer you to the podiatrist. 

Counselling

Dietician 

If your doctor feels it is appropriate to refer you for dietary advice they will arrange for you to see the dietician.
