Long term condition review

To help keep you safe and ensure your medication remains appropriate, we regularly invite patients with long-term conditions (such as diabetes, asthma, COPD, high blood pressure and other ongoing health conditions) for an annual review.

These reviews usually include blood tests (where needed), a health check and a review of your medication.

When will I be invited?

We will usually invite you for your annual review during your birth month. Patients born in January, February or March are generally invited during April, May or June.

We will normally contact you by text message first. If this is not possible, we may contact you by email, telephone or letter.

If we do not hear from you

If you do not book your blood tests or review, or you do not attend your appointment, we will make several attempts to contact you and encourage you to rearrange.

This may include:

  • Text messages
  • Emails
  • Telephone calls
  • Letters (where no other contact method is available)

What happens if I continue not to attend?

Regular monitoring is important to make sure your medication remains safe and appropriate. If you repeatedly do not attend or do not respond to our invitations:

  • Your prescriptions may need to be issued for a shorter period (for example, 14 days and then 7 days at a time).
  • We may ask you to collect your prescription from the surgery rather than having it sent directly to your nominated pharmacy.
  • We will continue to try to contact you to help you arrange the appointments you need.

These measures are taken to support safe prescribing and ensure we are monitoring your health appropriately.

Why are these reviews important?

Long-term condition reviews help us to:

  • Check that your condition is well managed.
  • Ensure your medication is safe and effective.
  • Identify any changes in your health as early as possible.
  • Reduce the risk of complications.

If you receive an invitation, we encourage you to book your appointments as soon as possible.

If you are unable to attend, please let us know so we can arrange an alternative appointment.

How do I submit my request?

  • are registered at the surgery
  • have been invited to do so
